Non-medicare patient and the codes submitted and, in my mind, should all be reimbursed are:
95134-for insects(one dose prepared, provided and administered)
95117-for trees(two injections given)
95165 X 40-for 40 doses of tree extract/2 doses given same day
Insurance paid 95134 and stated 95117 is included in 95134. I disagree and would like any input and/or shared experience with this scenario. I have read the CPT Assistant May 1996. The 95165 also denied.
