Procedure: Insertion of Impella 5.0, Left ventricular support device with Removal of left Femoral Percutaneous 2.5 Impella.

How would I bill a 33990 Insertion of VAD - if it was done as, Cut down/open?

Would it be 33990 with a modifier 22?

I think the removal of Impella 2.5 is Bundled into the insertion.

33990 and 33992-59 if it was a different site. If the insertion was at the same site then I believe you just bill the insertion. You can bill 34812 in addition if was a femoral cutdown

Yes, this was a replacement. The 34812 would be great but this was an Axillary cut-down. I spoke to someone from Abio-med. They suggest the 33999 Unlisted procedure.
 
yes. if it's the axillary then it is an unlisted code. just have it priced as the 34812. I had to do the same.
