Has anyone been paid for implants by MV carriers in 2013? What code are you using? My ASC was denied payment for L8699, prosthetic implant, for procedure code 28445 and 29827, separately. They stated it was inclusive.

The guidlines indicate that prosthetic and other devices including interna/external fixators, plates, screws, anchors, etc. are covered.

Any help would be appreciated.
