If an office is only doing an in house urine drug test does the office need a CLIA waived certificate. It uses a cup with result indicators on the cup. Most offices that have a CLIA certificate perform multiple labs so it seems the UDT would not require the certificate, right. Appreciate information from someone who is familiar with this matter. Thank you.
 
I am in KY and recently Caresource has branched from Humana. I am confused for the laboratory coding they are asking. We do have a CLIA waiver. Can someone please help me with this? I have denials. My director is wanting to add QW on the codes. I know we have ran into many issues with this. Is anyone else adding the QW, especially to 80307 with the G-code.
