I need to put together a quick guide for my physicians on documenting for balloon sinuplasty CPT 31295, 31296, 31297. Does anyone know the key elements that are needed to support these codes?
 
You are asking for templated documentation. I would think that whomever your physicians are purchasing their balloon system from, Acclarrant or Stryker, would have these templates for your providers. This is not something coders can provide. This is clinical documentation and comes from a surgeon. I could connect your office with another ENT office who I know who is doing BSD in the office if you would like so you could talk to them about their documentation templates.
