I've had basic in office lab work denials (blood lead test and hemoglobin) when billed by nurse practitioners. The denials say provider is not eligible to provide the service and denied as a specialty claim. I am only getting these denials from United Medicaid. I called United and of course they couldn't explain any further. Any suggestions?
