In-Office Ob/Gyn Ultrasounds

We our and Ob/Gyn office and we do in office ultrasounds for our patients; we wanted to find out if we need to have a separate note within the chart to support billing the total component for ultrasounds? For example as is needed in orthopedics when an X-Ray is done in the office to bill the total component you must be a separate and identifiable interoperation note in the medical record is that true in OB/Gyn as well?
