Wiki In patient E/M service with Echo Interpretation

We are getting denied the hospital subsequent visit on dates that the physician is reading the Echo that was ordered. Our hospital does not have staff Cardiologists so the on call physician reads the Echo and if it is for one of our patients then he is being denied the E/M code. These are commonly Medicare or Medicare advantage patients and I am trying to find a way to bill this so that they are both being reimbursed. Thanks for any input.
 
Did you put a 25 modifer on the subs.hospital visit? That could be why
it was denied.
