Incident to Service Private practice

Hello,

I was wondering does anyone know the Georgia Law for Incident to for mid-levels. I've been getting different information and not sure what some of it mean! I understand that the physician has to prove the initial visit for the patient and then the mid-level is allowed to see the patient. Is the physician required to document in the patient's chart as well?
