Wiki incident to billing for Anthem

I know that incident to billing per Medicare guidelines say that the supervising Dr. has to be physically in the office. A coworker of mine is saying that for Anthem BCBS, the supervising Dr. just have to be able to be reached by phone. I can't find this anywhere and we have a PA that was here by herself. I do not think we can charge this visit and she is saying we can. Does anyone know if the Anthem guidelines are different than Medicare?

unless you have it in writing from Anthem that the provider does not have to be onsite then you follow Medicare.
