We are mental health office with one MD and one NP. When I bill Incident-to Billing, do I put NP's NPI in box 24J, and MD's NPI in box 32 and box 33? We have a group NPI but cannot be used because it's not credentialed with most of the carriers such as Optum (UHC), Anthem and etc. Also does the documentation need to be signed by both MD and NP?
