Does the MD need to re-establish the plan of care for a patient that has been following incident to, if the MD was out of office at the last appointment? Example, Incident-to was established by MD on 1/1, incident-to requirements were followed at the 2/14 and 4/10 appointment and the MD was billed. At the 6/1 appointment the MD was out of the office and the APP independently saw the patient with no change to the plan of care, the APP was billed due to incident-to requirements not being met. At the 8/1 appointment, would the MD need to re-establish the plan of care in order to continue to bill for incident-to at future appointments?
