Can anyone provide documentation reference which states co-signatures by supervising physicians on ALL charts or just a sampling? MLN SE0441 doesn't specifically state ALL incident to services must be co-signed by the supervising physician. Can the physician co-sign a percentage of physician extenders charts? We have always had the supervising physician review every ob and return visit documentation, but wondered if this was necessary. I cannot locate any references for this. Thank you so much!
