We have a 2 physicians in our practice and 4 Pa's. When billing incident to can it be ANY provider that has initially seen the patient first? Should we bill commercial carriers also under the physician who is in the office that day?

Yes. any MD or DO can establish the plan of care.

However, any MD or DO can be on-site supervising. It doesn't have to be the MD or DO that established the care.

The MD or DO that is onsite that day is the provider that is billed on the claim under incident-to. (not the one that initially saw the patient)
