I have a question when billing incident to for a Medicare patient that is seeing a PA.

Senerio 1: A Physician Assistant is seeing a Medicare patient for a follow up and a new problem arrises. (Or a new Medicare patient sees a PA.) The PA goes and grabs the doctor to meet with the patient about this. What documentation is required to bill incident to? Can the PA document the doctors presence, face to face time and what was discussed in her office note? The doctor could also sign the PA's office note in agreement. Or does the doctor have to dictate a separate note entirely?
