Are there any books or online resources that can tell you what procedures are included in each CPT code, like if imaging or catheter placement is included? Or maybe something that describes the steps for each procedure?

It would be something like this book, only more up to date:

Decision Health has a book with the plan English definition of each CPT code - https://www.codingbooks.com/2016-plain-english-descriptions-for-procedures.html Looking at some sample pages this looks pretty good since there are some images

Some of the encoders have the Decision Health definitions as an add-on. I use findacode.com

There's similar options for ICD-10 and HCPCS
