Baby born at 29 weeks, came to see us for their first visit after her release from NICU at 2 months of age. The doctor requested they come back for a weight recheck since they were a preemie with low birth weight. They come back in, her note says the child is fine. She coded P07.15 low birth weight and P07.32 preemie. The baby came in for another visit afterwards and the same codes were used.

Am I able to code these as primary dxs, especially since the baby is past the perinatal period? There isn't another problem in the note, so I'm not sure what to put. The doctor does give anticipatory guidance on feeding and care.

Any suggestions? Thanks.
