Can anyone give me a dx code for a patient who is infertile due to chemotherapy treatment?

Look at using 2 codes: 628.8(infertility female of other specifide origin) and E933.1 (drugs correctly administered causing adverse effects--antineoplastic & immunosuppressive drugs--- cancer ) I don't know if you want to list the type of cancer or not as a 3rd code.
