Can someone tell me where I can find documentation stating that before the claim goes to the insurance company the chart needs to be signed off on?

The entire chart? Perhaps this is something new? This is the first that I have heard of the entire chart needing to be signed.

Since each medical record stands alone the documentation does need the providers signature and date before filing the claim. This rule pertains to the individual medical record, not the entire chart.
