I am not sure about the right code for this procedure. The only code we came up with is 23405 or to use unlisted procedure code.

Patient was placed in sitting position. Right shoulder tender on the lateral aspect of the humeral head and over the right joint. Ultrasound of this area revealed hyperechoic area involving the infraspinatus insertion onto the humeral head. After an alcohol prep a 25-gauge inch and a needle was directed into this tender area with reproduction of the patient's pain. The series injected 0.25% bupivacaine with 4mg per cc of methylprednisolone. This was followed by a tenotomy or the needle was moved in and out doing small lacerations of the infraspinatus tendon at its insertion onto the humeral head. Aproximately 30 fenestrations were performed. The needle was removed.
