In ED patient receive Pepcid and Zofran at same time. But I do not see it is 2 different sites (RT/LT arm) Is it possible they mix 2 drugs together? Or the nurse need the education for documentation .We can code for concurrent infusion??

Similar situation with Psych patients also I do see lot of medications (Haldol,etc.,) on same time with same issues mentioned as above.

your nurses need to document better if you cannot l=tell from their notations how many IV lines and where they are located, and whether it is a main bottle with a piggy back, or if it is two bottles concurrent. You cannot guess, and if you cannot tell then you cannot code it and that is lost revenue.
