Does anyone have a regulation or guidance regarding what is required to bill infusions? For example, can the documentation say the pateitn received infusion services from 0915 - 1430, and during that time received 3 separate medication IVPs (without notation of time administered), and had a 4 hour infusion of Cytoxan, and bill for the 3 IVPs and the chemo infusion (first hour, and 3 hours additonal?).

Good morning, I review & post Therapy infusion's, Chemotherapy infusion's, IV push's & hydration on an outpatient basis @ the hospital. We have a Hierarchy that we use as our guide.
