If you have no stop time documented for an infusion from the nursing staff and they have charged a 96365 can I default this to a 96374? I have been told that I can default to the 96374 and also have been told that if I don't have proper documentation to prove that the infusion was less than 15 minutes I can't bill for it. Which is correct? I have documented proof that this was done through the IV so if I can't default to the 96374 can anyone tell me why?
