If you audit an initial inpatient visit, and you get a 99221 for the history, a 99222 for the physical exam, and a 99223 for the MDM, how do you code this visit? I know it says you must have 3 of 3 elements. In that case, does this mean this is a non-billable visit?
 
You go with the least. You must meet or exceed the 3key components. So when you meet the level 1 with one component yet exceed the level 1 with the other two components, then you code to the level 1.
