I have a patient who had a head injury in 2003 and she hit her head again in the same spot as 2003, do I code the current visit with the re-injury as initial or subsequent?
 
It is a new injury, if you are providing active treatment use the A, if you are providing follow up care after the patient was treated elsewhere use the D
