I have a question about what makes up an initiating visit for CCM services. My personal interpretation of Medicare's description for an Initiating Visit is that the provider MUST discuss CCM with the patient for it to count as the initiating visit. That without this discussion, the visit is a regular E/M, AWV, IPPE visit and no CCM services can be started. But some other staff members say that they interpret this as saying that if the patient is an established patient, that they qualify for CCM services. I am new to billing for CCM services and want to make sure that I know what to look for and understand what Medicare requires. Any help would be greatly appreciated.
