injection coding - correct code

Can anyone tell me what is the correct code for injection of the lateral ankle gutter for capsulitis?
 
You may want to post the Op note so people can review it. Are you wanting to know the injection procedure or the medication?
 
"Injected Kenalog 10mg mixed 50:50 with Lidocaine 2%, 0.67ml total to the lateral ankle gutter where capsulitis symptoms were painfully palpated, right."

The doctor called it a 20600, but that is for a small joint and the ankle is an intermediate joint. I think he did that becaus he was also billing 20605 x 2 for bilateral injections into other areas of the ankles. So, what would 20605 x4 look like? Patient has Humana Gold MCR replacement.
