My provider is injecting a mixture of vitamins in 6 separate sites (3 Rt. leg and 3 Lt. leg) with 6 separate syringes. Each syringe contains the following mixture.
J3420 B-12 9mcg hcpcs code dosage is 1000mg
J3415 Pyridoxine 0.45 mg hcpcs code dosage is 100mg
J3411 Thiamine 0.45mg hcpcs code dosage is 100mg
J1955 Levocarnitine 0.0022gm hcpcs code dosage is 1gm

In this case would I treat each of the injections/syringes as a separate dose or would I add up the contents of all of the injections for dosage?
