We are having issues with the insurance companies denying therapeutic injections when they are billed more than once.

For example ..they come in for a problem and then they additional need a testosterone and a B12 injection

So we are billing 99213 mod 25 for maybe 401.9 , 250.00
J1080 restrict DX 257.2
J3420 restrict DX 266.2
96372 X 2 Dx 257.2 , 266.2

Insurance is denying the injection even when we key them in like this
J1080 DX 257.2
J3420 DX 266.2
96372 DX 257.2
96372 DX 266.2

Am I needing to put a MOD 59 on second injection ? I didn't think we would.
