If patient was previously treated for DJD right knee with injection. Returns and now has new diagnosis of prepatellar bursitis right knee. The knee is injected. Is the bursitis considered a new problem? May I charge an OV for injection of bursitis?

It depends completely on the office note and what was documented that was relevant to the presenting issue(s)". A new problem does not guarantee that the note will meet the criteria of significant and separately identifiable.
