What information can you give regarding injections and the drug?
I am having difficulty finding solid guidelines.

My thoughts are, there should be an order for the drug, a brand name in the notes, as well as a batch number in case of recall. Also the method of injection.

I see vague discriptions, i.e. Testosterone, 100ml, SQ....

According to the NDC list, there are 3 brands of this drug. Without more information, which NDC number is reported to the insurance company?

I pull the NDC off the vial that was used to administer the injection along with lot number and expiration so there is a complete record. If you are in a large practice with lots of injections/providers, then you may want to use a better tracking system and make sure everyone is on board with this.
