If a patient has a procedure (i.e. injection, PFT, etc.) - that is done by an RN or MA, is it necessary to have all the documentation in the original note? Or could you submit the procedure, done by an MA, on a separate sheet/note?

I understand it's easier that have it all in 1 note but is there a compliance issue?
