If someone comes in with an injury or pain and the findings of the report end up being DJD which do you code first? The injury/pain or the DJD?:confused:
 
When a definitive diagnosis is not identified you would code the symptoms. If at the end of the exam the provider documents a definitive diagnosis you should code the diagnosis.

Remember no rule outs on out patient services, symptoms or diagnosis only.
