If a Fellow does the consult but the Attending doesn't sihn off on it until the next day, which DOS do you charge for and is it consult or subsequent?

When did TP SEE the patient

The DOS is the date the teaching physician actually saw the patient ... even if s/he didn't get around to co-signing the fellow/resident note till the next day.

Check with the provider.

If the patient is covered by Medicare (or another carrier who is following Medicare guidelines on consults), then you would charge an initial hospital visit (assuming this is the first time the patient is being seen in this hospitalization).

If the patient is NOT covered by Medicare (or another carrier who is following Medicare guidelines on consults), then you bill the consult like you always did.
