Hi everyone! We are a MD office with hospital rights. We bill on professional claim forms. I have a situation where a patient was inpatient hospital from 2/2-2/6, then after discharge on 2/6, returned to the hospital for another 4 days before passing away. We had already submitted a discharge claim for 2/6, and now I don't know how to bill the initial for the same day! Do I send a corrected claim for admission/discharge for the 6th? The patient also had advanced care planning 99497 on this same day. Also, the day the patient passed away is that billed as a discharge? Thank you!!!!! I appreciate any guidance I can get for this one!
