I have a consult code that dropped for my cardiologist. The patient has Medicare so I know I can not code the consult, I have to change it to either a 99214 or a 99204 depending on if we have seen the patient in the office. She is still listed as observation and tonight would make my 2 midnight rule. My question is, if the patient is changed over to inpatient in the morning, because the consult was done on the 14th, do I still charge the 99214/99204 or do I change the code to an initial hospital code 99222?
