When a patient comes in to get an INR check does the doctor need to document the visit? The patient is not seeing the doctor only the CMA.

Payer like Medicare only reimbursed if it is medically necessary for example-long term use of anticoagulant. ABN must be signed by the patient and claim should be billed with modifier QW. No provider note is needed for INR. Only signed document from the lab professionals. Need physicians orders/ prescription for the testing.
