does anyone know what the code would be when the anesthesiologist does this during a heart case?

I would use one of the CPT codes of 33224 and33202 for pacemaker insertion. However if fluoroscopic guidance is required use code 76000;if not then do not use that code 76000. Also since the anesthesiologist is doing the procedure of inserting a pacemaker wire/electrode use two modifiers 59-distinct procedure since he normally does not do this process and modifier AA designated done by anesthesiologist. Also modifier 59 can be used for anesthesia and surgery settings.
