Often coders are directed by their supervisors or team leaders to interpret coding instructions based on the team leadership decision. Coders adhere to these decisions although they may disagree and sometimes voice their opinions on the topic. Of course the general idea is to follow the business decision of the leaders in the coding department.
Here is one that came through in an email. Understand the scenario. The patient had a procedure performed under anaesthesia. After the procedure the patient was admitted to inpatient care for post operative pain management. The performing surgeon requested a consult for the patient from the pain management provider on the same day of the procedure while the patient is inpatient status.
This is the coding instruction that followed. Examine it and understand the consequences of what could possible happen from a reimbursement stand point when you bill for post op day 1 and days following for post operative pain management.

If patient is seen by pain management consult on the same day a surgery was perform and patient was also seen by the anesthesiologist- suggest to code 99024 (Post-op follow-up visit). Pain management consult is included in surgical package."
