Can anyone tell me how far back an carrier can go to recoupe an overpayment or that the carrier feels that they should have not paid?
 
It will depend on your state and whether the plan is governed by state laws or federal laws.

In Louisiana, plans that are governed by state law are allowed to go back the same amount of time that they allow for timely filing.

For example, United Health Care allows 90 for their filing time limit. So, if they want to recoup, they must request the refund within 90 days from the date of their payment.

You should contact your state insurance commissioner for more info.
