Insurance terminated during hospital stay

I have had Cigna deny our surgical claim for services rendered after termination date. The patient was admitted to the hospital on 06/25/14 and not discharged until 07/08/14. Her plan terminated 06/30/14. Our surgery date was 07/01/14. Hospital got pre-admission certification and got paid. Our surgeon didn't get paid for his services done 07/01/14. I cannot find anything that addresses continued care in this situation on Cigna website. Does anyone have information on this or had experience with it?

I appreciate any help.
