Wiki Intake code for behavioral health provider

We are a behavioral health provider.
We are looking for a CPT code to bill for intakes for new patients that does not require licensed clinician. The intake will include

1. Medical history
2. Social history
3. Substance abuse history
4. Psychiatric history
5. Reason for seeking treatment
6. ‘history of presenting illness
7. PHQ, GAD and PSS3 scores
8. Current treatment being provided
9. Educational history
10. Developmental milestones with normal limits
11. Reaction to medication in the past
12. Previous response to treatment
13. Mental status

This intake would not require goals or diagnosis of the new patient. The patients first visit with the clinician would determine goals and diagnosis.

Does anyone have any tips on this?

I could be mistaken but you cannot bill for BH unless you have the proper license.

I believe that the intake would be included in any services that will be billed.
