DOES ANYONE KNOW THE REASON OR THE MEANING BEHIND THE WORK "each" with this code? Is it referring to the number of ?? or does it have something to do with the third party payor wanting the info?? please help????

It means for each additional lesion, it's an add on code.
Example: Destruction of 5 premalignant lesions....
17000 (first lesion)
17003,17003,17003,17003 or 17003 X 4 (lesions 2-5)
