Good afternoon,

I am auditing TCM cases and I have one case where the patient was discharged from the hospital and Home Health was initiated. I do not see where interactive contact was made within 2 business days of discharge outside of Home Health contacting the patient. My question is with HH contacting the patient does that count as credit towards the interactive contact for TCM? Or should there been additional contact specifying Transition of Care purposes? Any help with this would be greatly appreciated. Thanks
