Provider removed a basal cell carcinoma on a patients lower leg. Based on the documentation, I believe this is an intermediate repair and not complex repair. Please advise.
Excerpt from op note:
"Sharp dissection was taken down to the fascia layer and lesion with margins removed with electrocautery. Hemostatis obtained and wound irrigated with warm saline. There was large defect 6cm x 12.5 cm on the anterior aspect of the lower leg which could not be approximated for closure, therefore using metzenbaum scissors the skin and subcutaneous fat was elevated off the fascia layer in all directions. Complex closure was begun at each end of the wound using 2-0 Vicyrl sutures. A 4cm x 6cm open area to the central portion of the wound could not closed despite freeing up the skin and subcutaneous fat off the fascia. Decision was made to apply a wound vac in this area."
