In the intermediate repair section, it states that "single layer repair closure of heavily contaminated wounds that have required extensive cleaning OR removal of particulate matter also constitutes intermediate repair"
What exactly does "extensive cleaning of particules mean? It is not exactly specific as to when the coder can really do this.
EX. 1
each laceration cleaned. pieces of blue paint chips removed from both lacerations. suture of 4.0 ethilon placed in 1 layer closure.
Is this intermediate? or stay with simple?

rust FB removed with addison pickups as well as irrigation. wound close with 4.0 proline. This is simple layer repair.
Can this be in the intermediate repair??
If any one has info on this, L would love to have input.
Currently all are treated as simple repair, not intermediate.
