Wiki Interstim - would I code this

After a recent Interstim placement, the pocket the Interstim was in was not healing properly. The surgeon moved the Interstim generator from that pocket on the right side to a newly created pocket on the left side. He then re-tunneled the electrode from the right side to the left side and re-connected it to the Interstim. How would I code this? Should it be 64595 and 64585 or would it be 64590?
