I am in need of help determining a CPT code for a procedure on an ESRD patient. "Angioplasty of stenosis of the juxta anastomosis of patient's left upper arm transposed brachiobasilic AV fistula." Provider turned in 35476 but I am thinking it should be 35475.
 
Unfortunately, I would need to see the entire OP note to suggest the correct code to you. If the procedure was of the "true brachiocephalic" branch, then the correct code is 35475. If this was in the venous portion of the brachiocephalic arch, then it would be 35476.
