When a provider uses the same code for multiple encounters how do you address the provider?

How do you answer this question for an interview?

this question would require more information, but I would say, the code needs to supported thru the documentation. if the documentation doesnt support what is being billed, the provider would need to be questioned and possibly trained on correct coding/documentation
