I am billing intralesional injections for morphea with CPT code 11900 and ICD-10 code L94.0. A specific ins co is rejecting these saying that the codes are not compatible with published evidence-based coding literature. The rejection letter says that there is a more appropriate code combination for this procedure and diagnosis. I cannot find anything different than what I have billed. I've sent notes as well as literature on morphea. My second-level appeals were denied as well. What other codes could I possibly use for this?
